Sometimes it is difficult to know what questions to ask and where to start. Speak with a trained Care Consultant to address your unique concerns and prepare for the future. Consultations are confidential and free of charge.

 

Care Consultation can assist you and your family in creating a road map to navigate through your many thoughts, emotions, and questions. Care Consultation is useful whether you or someone you love has just been diagnosed or if you have been impacted by a diagnosis for some time. You will receive valuable one-on-one assistance that will enable you to better understand the disease, manage the symptoms, and live life today. Consultations take place in-person and/or by phone depending on your location and preference and include assistance in:

 

  • Identifying your current needs and goals
  • Developing an action plan
  • Problem-solving surrounding symptom management
  • Learning about the disease and anticipated progression
  • Locating resources and services specific to your needs
  • Ongoing support and follow-up as requested
  • Helping those who live alone
